Qualification

Jury staff of the superior courts do not ask past or prospective jurors for information regarding credit card, bank account information and Social Security numbers. Please do not provide this type of information to anyone claiming to be associated with Jury Services. Please contact our office if you receive this type of request.

Qualification/Disqualification

In accordance with Code of Civil Procedure, Sec. 203 (a), all persons are eligible and qualified to serve as prospective trial jurors, except persons with circumstances listed below. Click on the link that best describes your circumstance and follow the steps to complete your questionnaire. If you are certain you meet the requirements to be disqualified, you may access the eJuror system to request a disqualification.

Remember to sign your name and date the affidavit on both the front and back in the spaces provided. Also write the city where you executed the affidavit under your signature on the back of the affidavit. If you are completing the affidavit for another person, write your relationship to that person after your signature on the back of the affidavit.

I am able to serve
YOU ARE ABLE TO SERVE AS A TRIAL JUROR

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Check the box next to the red text that states: I DO NOT MEET ANY OF THE DISQUALIFICATION CRITERIA AND AM QUALIFIED TO SERVE AS A JUROR
  3. Sign, date and write the city where executed
  4. Mail the Affidavit with the correct postage
I AM NOT A CITIZEN OF THE UNITED STATES

If you are not a citizen of the United States, you are not qualified to serve as a trial juror.

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Check the box next to A. AM NOT A CITIZEN...
  3. Write the country of citizenship.
  4. Sign, date and write the city where executed
  5. Mail the Affidavit with the correct postage.
I AM UNDER EIGHTEEN YEARS OF AGE

If you are not 18 years of age, you are not qualified to serve as a trial juror.

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Check the box next to B. AM UNDER EIGHTEEN YEARS OF AGE.
  3. Sign, date and write the city where executed
  4. Mail the Affidavit with the correct postage.
I AM NOT A RESIDENT OF ORANGE COUNTY

If you are no longer a resident of Orange County, you will need to tell us where you now reside. The Jury Commissioner’s Office receives its list of names from Voter’s Registration and the California DMV. Make sure you have updated your information with those agencies.

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Write your new address in the address correction space on the front of the questionnaire.
  3. Check the box next to C. AM NOT A RESIDENT OF ORANGE COUNTY
  4. Sign, date and write the city where executed
  5. Mail the Affidavit with the correct postage.
I DO NOT HAVE SUFFICIENT KNOWLEDGE OF THE ENGLISH LANGUAGE

If you have been convicted of a felony or malfeasance in office and have not had your civil rights restored, you cannot serve as a juror. If you are unsure, contact the court where your proceeding was held or consult a legal professional.

  2. Check the box next to E. HAVE BEEN CONVICTED OF A FELONY OR MALFEASANCE IN OFFICE...
  3. Sign, date and write the city where executed
  4. Mail the Affidavit with the correct postage.
I HAVE BEEN CONVICTED OF A FELONY OR MALFEASANCE IN OFFICE

If you are/or will be serving as juror in any court.

  2. Check the box next to F. AM CURRENTLY SERVING...
  3. Check the line next to GRAND or TRIAL JUROR
  4. Sign, date and write the city where executed
  5. Mail the Affidavit with the correct postage.

Note:
If you have served as a juror in the last 12 months at the United States District Court, or Federal Court, you can request to be excused from jury service at the Superior Court. However, the Federal Court will not excuse you unless you have served as a sworn juror within the last 12 months.

I AM SUBJECT OF CONSERVATORSHIP

If you are subject to a conservatorship.

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Check the box next to G. AM SUBJECT OF CONSERVATORSHIP
  3. Write the name of your conservator.
  4. Sign, date and write the city where executed
  5. Mail the Affidavit with the correct postage.
I HAVE A PHYSICAL OR MENTAL DISABILITY

If you are not able to serve due to a physical or mental condition.

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Check the box next to H. PHYSICAL OR MENTAL DISABILITY..
  3. You must send a doctor’s verification (note) of a condition that prevents you from serving UNLESS you are 70 years of age or older.
  4. If you are 70 years or older, check the box next to H. and provide a brief description
  5. Sign, date and write the city where executed
  6. Mail the Affidavit with the correct postage.
I HAVE A PERSONAL OBLIGATION TO GIVE CARE TO ANOTHER

If you provide care to another, it must be on a full time basis. The care must be a PERSONAL obligation not a PROFESSIONAL obligation.

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Check the box next to I. CARE TO ANOTHER...
  3. Explain your obligation.
  4. If employed, you MUST include your work hours.
  5. Sign, Date and write the city where executed
  6. Mail the Affidavit with the correct postage.
I WILL BEAR AN EXTREME FINANCIAL BURDEN

If you will bear an extreme personal financial burden by serving jury service.

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Check the box next to J. EXTREME FINANCIAL BURDEN
  3. In the first space provided, write the number of dependents you have. In the second space provided, write the estimated monthly net income for your household.
  4. In the third space, write the estimated monthly expenses for your household.
  5. You must provide us with this information to be excused for extreme financial burden as well as having no days paid for jury service by your employer. Failure to provide this information will result in a denial of an excuse under this request.
  6. Self-employment or having an employer that does not pay for jury service is not an automatic excuse.
  7. Sign, Date and write the city where executed
  8. Mail the Affidavit with the correct postage.
I HAVE SERVED AS A JUROR IN ORANGE COUNTY WITHIN THE PAST 12 MONTHS

If you have served as a Grand Juror or Trial Juror within the past 12 months, you are excused from serving at this time.

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Check the box next to K. Prior service.
  3. Write the date of your last service in Orange County.
  4. Sign, date and write the city where executed
  5. Mail the Affidavit with the correct postage.
THE PERSON SUMMONED IS DECEASED

If you are completing the summons for a person that is deceased.

  1. Check the box next to N. This person is deceased
  2. Sign, date and write the city where executed
  3. Next to your signature, write your relationship to the person summoned.
  4. Mail the Affidavit with the correct postage.
I AM A PEACE OFFICER

If you are a peace officer, you may be exempt from serving as a trial juror.

  1. Fill out the Front of the Juror Affidavit Questionnaire
  2. Check the box next to R. The peace officer exemption.
  3. Not all peace officers are exempt, please refer to the stated codes.
  4. Sign, date and write the city where executed
  5. Mail the Affidavit with the correct postage.
